1* How closely have you followed recent news reports about autism?

2*  How serious a problem is autism in America today – very serious, somewhat serious, not very serious or not at all serious?

3*  Which is the more likely cause for the increase in the number of children diagnosed with autism – genetic factors, environmental factors, certain vaccinations or increased awareness and detection?

4*  Who is most responsible for dealing with an autistic child – parents, schools,  or government agencies?

5*   Have you or anyone in your family been diagnosed as autistic?

6*  Do you have children under the age of 18?

7*  (Asked only of those who say yes to #6) Has your child received all the vaccinations that he or she is supposed to have?

8*  How concerned are you about the safety of vaccinations for children?

9*  How concerned are you that children who do not receive all their vaccinations will cause health problems for other children?
